About the role
The Home‑Based Care Community Health Worker (CHW) supports patients and their families at the community level. Working as part of a multidisciplinary clinical team, the CHW provides health education, psychosocial support, and serves as a vital link between the clinic and the community.
Key responsibilities
- Increase awareness about TB, HIV and available services through community sensitisation activities.
- Conduct home visits to educate patients, provide psychological and social support, and identify health needs.
- Record patient information accurately on CHW forms and report to clinic staff.
- Make referrals to appropriate clinic personnel when additional care is required.
- Collaborate with doctors, nurses, pharmacists and social workers to ensure coordinated care.
Required profile
- Strong communication skills and ability to engage community members.
- Willingness to travel across Lei, Tankoro, Gbense and Kamara chiefdoms.
- Basic literacy and numeracy for accurate record‑keeping.
- Team‑oriented mindset with empathy for patients and families.
Required skills
- Accurate manual data collection and form completion.
- Basic health education techniques.
- Ability to use simple community‑based health tools.
