Community Health Worker – Home Based Care

About the role

The Home‑Based Care Community Health Worker (CHW) supports patients and their families at the community level. Working as part of a multidisciplinary clinical team, the CHW provides health education, psychosocial support, and serves as a vital link between the clinic and the community.

Key responsibilities

• Increase awareness about TB, HIV and available services through community sensitisation activities.
• Conduct home visits to educate patients, provide psychological and social support, and identify health needs.
• Record patient information accurately on CHW forms and report to clinic staff.
• Make referrals to appropriate clinic personnel when additional care is required.
• Collaborate with doctors, nurses, pharmacists and social workers to ensure coordinated care.

Required profile

• Strong communication skills and ability to engage community members.
• Willingness to travel across Lei, Tankoro, Gbense and Kamara chiefdoms.
• Basic literacy and numeracy for accurate record‑keeping.
• Team‑oriented mindset with empathy for patients and families.

Required skills

• Accurate manual data collection and form completion.
• Basic health education techniques.
• Ability to use simple community‑based health tools.
