About the role
Partners In Health is seeking two Community Health Workers to join its community‑based program in Sierra Leone. The role involves providing home‑based care, health education, and psychosocial support to patients and families while acting as a vital link between the clinic and the community.
Key responsibilities
- Conduct home visits to educate patients and families about TB, HIV, and other health services.
- Lead community sensitization activities to reduce stigma and increase awareness of available health services.
- Provide psychological and social support to patients, referring them to appropriate clinic staff when needed.
- Accurately record patient information on CHW forms and maintain up‑to‑date community health data.
- Collaborate with doctors, nurses, pharmacists, and social workers as part of a multidisciplinary clinical team.
Required profile
- Strong commitment to serving poor and marginalized communities.
- Ability to work independently in remote chiefdoms and travel to households.
- Good communication skills for community outreach and health education.
- Willingness to seek guidance from team members and refer patients appropriately.
